2. Medical Release Form (see below) notarized
3. Player Membership Form (see below)

In addition, you need to provide your manager with a copy of your Birth Certificate and one 1"x1" photo (on photo paper)

2010 - 2011 Medical Release Form * - (required for all registrations) NOTE: Must be notarized.
